Dr. Erik Zamboni, DPT, CSCS,CEAS (aka 'Dr. Z'): Dr. Z earned his clinical Doctor of Physical Therapy degree (DPT) from the University of Southern California (USC) in 1999. USC continues to be ranked the number one physical therapy education program and has been for most of the past decade.
Overall, his professional training amounts to twice the Oregon state requirements. In terms of work experience, Dr. Zamboni has over 17 years experience including opening a clinic for the national leader in occupational health, Concentra Medical, as well as directing an outpatient orthopedic clinic for one of the Forbes ranked top small businesses in America. He has been the Clinical Director of Living Well Therapy, LLC since 2004.
Dr. Zamboni once spent two summers living in the Amazon jungle and with that experience when combined with a minor in Spanish, he is fluent in order to meet the needs of a more diverse population. Dr. Zamboni is a member in good standing of the Oregon Physical Therapy Association and the American Physical Therapy Association.
In his free time, he enjoys camping, fishing, hunting, hiking, international travel and most other outdoor recreation!


Heidi Washenberger:
Has over 30 years of experience as a physical therapist. She completed her physical therapy education at University of California, San Francisco/San Francisco State University. She has completed numerous continuing education courses in manual therapy and functional rehabilitation. Heidi has also completed her strength and conditioning certification (CSCS) from the National Strength and Conditioning Association (NSCA). Over the course of her career she has enjoyed working with a variety of patients to help them return to their sport or activity of choice with emphasis on using her manual therapy skills as well as a functional approach to help them achieve their individual goals.
Outside of working as a physical therapist:
, you may find Heidi running along the trails of Bend, enjoying the mountains while skiing or hiking or on her mountain bike. She also enjoys spending time with her two adult children whenever possible.
Anthony Izzo:
Anthony is a Physical Therapist Assistant in Bend, Oregon. With a passion for movement and an active lifestyle, he brings a hands-on approach to rehabilitation, ensuring each patient receives personalized care tailored to their recovery goals.
Outside of work, Anthony embraces the adventurous spirit of the Pacific Northwest. He can be found carving down slopes on his snowboard, hiking scenic trails, or enjoying local brews and food. His enthusiasm for movement extends beyond the clinic, inspiring patients to get back to the activities they love with renewed strength and confidence.

Sylvester William (aka The Sly) | Virtual Staffing Specialist
Sylvester joined Living Well Therapy as Financial Assistance Manager, expertly guiding patients to resolve outstanding balances, and as Digital Communications Manager, optimizing the website and amplifying the clinic's online presence.
Stepping up during a transition, they assumed the Lead Patient Care Coordinator (PCC) role, developing comprehensive training programs for seamless onboarding. Today, they train new PCCs while spearheading a innovative staffing agency, connecting healthcare PT clinics with cost-effective virtual assistants—our expertly trained PCCs—to streamline operations and reduce expenses.
Outside work, Sylvester stays energized on the basketball court, chasing waves in the ocean, and plunging into thrilling scuba dives—what a splash of adventure! 🌊🏀

I recently graduated from Texas State University with a degree in Exercise Sports Science, concentrating in Pre-Rehabilitation Studies. Originally from San Antonio, Texas, I’ve recently moved to Bend and absolutely love it here.
In my free time, I enjoy snowboarding, paddleboarding, and crocheting. I aspire to become a Physical Therapist and am eager to deepen my knowledge in this field while helping patients achieve a pain-free lifestyle.

As a Lead Auditor and Communication Specialist: I'm dedicated to making sure all medical records are completed in a timely manner and in patient's charts, maintain communications with other medical offices so we are working together in the best interest of our mutual patients, audit patient’s plans of care to help them get the results and goals they came for, and audit patient accounts to keep balances up to date and accurate.
Beyond my professional endeavors, you can often find me on the cricket ground. My favorite subject is Mathematics. I love to learn and teach Mathematics. I am married and have four kids, so I love to spend free time with my family.
